NOTE: All participants must have a completed physical form completed and signed by your physician brought to camp at registration on the first day of camp. No player will be allowed to participate until this item is submitted at registration.

I hereby authorize the staff of the Highlander Youth Soccer Camp to act for me according to their best judgement in any emergency requiring medical attention, and I hereby waive and release Cairn University and Highlander Soccer Camp and its staff from any and all liability for any injury. All medical expenses incurred will be the responsibility of the camper or the camper's family. I understand that any medication to be administered to a child during the camp must be administered by a parent, other adult or sibling duly authorized by the parent in writing (Please contact the director for details). The camp is not responsible for personal items that are lost, stolen, or damaged. I understand the camp retains the right to use any photograph, videotape, motion picture recording or any other record of the event for publicity, advertising or any legitimate purpose. I understand and accept the camp's no refund policy after June 30 and that the fee includes a $100 non-refundable portion.
